One time we ended up performing the Psychiatrist Fantasy as a puppet play with plushies. Big Green Plush Cthulhu as the Mustache-Twirling Psychiatrist, Small Red Plush Cthulhu as the Blonde Woman, Robin Hood Fox Plushy as Lesbian Nurse #1, and Empty Beer Bottle as Lesbian Nurse #2 (we ran out of plushies).

Sadly we gave our copy away to friends without realising how difficult it would be to replace.

Nope. I'm not sure if they're in the sections available on Google Books, and I'm really not inclined to buy the thing. :)
